Documents Required : Any one of the following Original documents are required at the time of booking for verification. Continuous discharge certificate (CDC) Passport Voter ID Pancard Driving Licence School Leaving Certificate Birth Certificate Students should produce copies of any one of the above mentioned documents for each course and 2 copies of passport size coloured photographs for each course. Medical Fitness Certificate : Candidates should undergo the medical check up (Arranged by our Academy), by a DG approved Medical Practitioner. This is required for Personal Survival Technique course and Fire prevention and Fire Fighting course. All candidates seeking admission to the Academy are to be in possession of INDOS numbers in accordance with Ms notice 02 0f 2001. The Academy will arrange to obtain INDOS from INDOS CELL through online. INDOS fee RS. 1300/- (Rupees one thousand three hundred only) is charged in addition with the course fee. This fee should be paid Along with the course fee. RULES FOR CANCELLATION & REFUND FEES : Once seat is booked, it will not be neither transferred nor adjusted with other courses. Cancellation more than 7 days prior to commencement of course, deduction 50% of the course. No refund whatsoever if cancellation is done within 7 days prior to booking. Candidates reporting late will not be admitted for the course. Course timings 0930 hrs to 1630 hrs from Monday to Saturday. QUALIFICATION AND ELIGIBILITY OF STUDENTS : This course is principally intended as basic training for seafarers employed or engaged in any capacity on board ship as part of the ship's complement with designated safety duties in operation of the ship. There are no particular educational requirements.
